When you call for an appointment the first time, we request that you print and complete our new patient form listed on our website under New Patient Forms. You may either drop off the form or fax it to us at (812) 373-2720. Our scheduler will inquire as to the nature of your needs. The information you provide will enable us to schedule the appropriate amount of time with your provider. Please try to select one of our providers as your preferred choice and schedule all appointments with this provider. Other providers offer acute services for you if your primary provider is unavailable. Please arrive 15 minutes prior to your appointment so that we may have time to check your information and have you ready to see the provider at the appointment time. Please provide your insurance card at each visit as well as your driver’s license at your first appointment visit. Please inform us of any changes in your name, address or telephone number.
If you cannot keep your appointment, please call during regular office hours to cancel. Our phones are answered Monday through Friday from 8am-5pm and are closed from 12pm to 1pm. The courtesy of canceling an appointment will allow us to use this time to care for other patients. We require 24 hour notice or you may be charged a $25 a fee. If you are assessed this fee, it is required to be paid prior to your next visit.
We require no smoking, no perfumes and no cell phone usage in the office. |
