Prescription Refills

If you need a prescription refill, you may do one of the following:

• Make an appointment to see a provider.
• Contact your Pharmacy and ask them to request a refill on your behalf.
• Please call our Pharmacy Refill Line at (812) 373-2700, Option 2.  Please be prepared to provide the following information:

• Name
• Date of birth
• Telephone number
• Medication
• Strength
• Directions
• Pharmacy Name & Location
• Your Sandcrest provider

We are sorry that we can no longer accept requests at our front desk.

It is best to request refills during your office visit.  If you have not seen your provider within the recommended time for follow-up, you will need an appointment prior to authorizing refills.  It is our policy that no controlled substances will be ordered after hours or on weekends.
